Going Home Program
The Going Home Program assists seniors in their own home following a hospital stay or emergency visit. This support helps seniors get back on their feet safely, by ensuring an appropriate level of care following a hospital stay.
This year alone, the Going Home Program:
- Served 1,200 clients at home
- Reduced the number of hospital inpatient days by a minimum of 1,200
- Prevented an estimated 3,000 inpatient hospital days as well as hundreds of return emergency room visits
- Received a 40% increase in the budget for 2010/11, bringing the number of clients to be served to grow by 50%
