Going Home Program
The Going Home Program is a free, community-based support service to help seniors and physically disabled adults return home safely following a hospital stay. This program is available for a maximum of 32 hours within a 10 day period after discharge and is provided in a clients' home. The Going Home Program is intended to help patients feel safe, secure, and cared for in their own home during transition period.
Services available include: personal care, caregiver respite, transportation, Meals on Wheels and light housekeeping. Referral to this program must be completed in hospital by a discharge planner, social worker, nurse, hospital case manager, or GEM nurse. Clients cannot self-refer.
This year alone, the Going Home Program:
- Served 1,200 clients at home
- Reduced the number of hospital inpatient days by a minimum of 1,200
- Prevented an estimated 3,000 inpatient hospital days as well as hundreds of return emergency room visits
- Received a 40% increase in the budget for 2010/11, bringing the number of clients to be served to grow by 50%
