At Carefor Health & Community Services, we are dedicated to delivering high-quality home health care programs and community support services, all of which are individualized to suit the specific needs of the people we serve throughout Champlain district.
Locally-managed, Carefor Health & Community Services is a not-for-profit, charitable home health care leader. Fully-accredited by Accreditation Canada, our organization has been helping those in need for over 110 years.
We work hard to provide services throughout the Champlain District. Carefor has three divisions: the City of Ottawa, Eastern Counties (encompassing Stormont, Dundas & Glengarry and Prescott & Russell) and Pembroke-Renfrew County.
Carefor Health & Community Services offers a wide basket of services including: Day Programs, the Guest House, retirement residences such as the Assisted Living facility located in the Pembroke Civic Complex and the Richmond Care Home. These services have become indispensable resources for clients living with Alzheimer's and other related dementias. Specializing in seniors' services, palliative care, support programs for the homeless and mental health and wellness clinics are also key areas that have been well developed within our organization.
We are a community leader and we partner with many other health care agencies to share expertise and offer support in planning the delivery of services to the community. Some of our current joint projects include: planning supportive housing projects, developing a coordinated transportation network for the Champlain District, seniors' services (Going Home and Regional Emergency Geriatric Service) and opening a new 10-bed palliative care hospice.
Our Board of Directors is made up of community-focused leaders who volunteer their time to promote the health and well-being of our organization. The Board leads by setting the strategic directions, focusing on larger issues, providing clarity to management, actively monitoring the quality of services, operations and fundraising and rigorously evaluating accomplishments through eight Standing Committees.
Core staffing at Carefor includes: personal support workers, nurses, occupational therapists, physiotherapists, dietitians, social workers, clinicians, program and residential care staff and administrative staff. We strongly believe in actively supporting continuing education opportunities for all of our staff and volunteers. Affiliation with universities, community colleges, the Alzheimer Society and Bruyère Continuing Care, to name a few, is fostered to ensure the development of best practices in all services.
Our proactive take over the past few years on fine-tuning our services has been eventful, successful and rewarding. I would like to take this time to personally thank all of our supporters for their continuing support.
