Summary of Function:

Our Registered Practical Nurses will provide quality nursing care to residents with the goal of helping individual residents attain an optimal level of independence and well-being. Medication administration is a key component of their responsibilities in addition to providing leadership to the Care Team through teaching and delegation.

Qualifications and Experience:

  • RPN Diploma from an approved college
  • Valid registration with the College of Nurses of Ontario (CNO)
  • Current First Aid & CPR Certification
  • Certificate in gerontology would be an asset
  • CPI, GPA or similar training would be an asset
  • 3-5 years’ relevant experience
  • Experience working in a retirement, nursing home or similar environment
  • Experience working with seniors or adults with dementia, Alzheimer’s, and mental health illnesses
  • Experience in handling confidential information

Knowledge, Skills & Abilities:

  • Ability to work a variety of shifts, in a 24/7 operation - days, evenings & weekends
  • Ability to observe and assess health care levels according to level of practice
  • Ability to perform duties according to the RPN scope of practice
  • Demonstrated leadership and teaching skills in a health care setting
  • Effective verbal and written communication skills
  • Develop individualized care plans and interventions as determined by the need of the resident.
  • Dependable, trustworthy, compassionate with good work habits
  • Ability to problem solve and to adapt nursing to independent practice
  • Ability to work collaboratively as a member of an interdisciplinary team
  • Ability to work a variety of shifts within the care team schedule
  • Ability to work independently in a busy, fast paced environment and prioritize duties
  • Effective problem solving and decision making skills
  • Ability to bend, sit, stand, kneel for short periods of time

Responsibilities:

  • Provides complete and relevant assessments of the resident and describes and/or documents findings according to procedures.
  • In collaboration with the Manager or designate, prepares and updates the resident profile and care plan and works with other members of the health care team to ensure residents receive quality care.
  • Effectively and safely administers medication according to Carefor procedures and CNO standards of practice.
  • Identifies problems using a specified framework and in consultation with the Manager or designate and resident, selects and implements individualized strategies or interventions in order to focus care on resolving or managing the problem.
  • Acts as a clinical resource and mentor for other care providers, staff and students.
  • Performs basic personal care, as required.
  • Provides cost-effective, organized, outcome-directed care on an ongoing basis.
  • Identifies and utilizes tools and resources for work and manages these resources through regular monitoring of equipment etc. and reporting unusable equipment to supervisor or designate.
  • Delegates care appropriately.
  • Assists residents to/from the dining room, as required.
  • Reviews all forms of documentation and communication in order to remain current on residence operations and resident care. This may include reviewing care notes, e-mails, memos, communication book, and receiving verbal updates.
  • Utilizes adult learning principals and established teaching materials to provide health teaching to residents.
  • Completes documentation according to established procedures to maintain the communication to other health team members and provides an accurate account of service provided.
  • Describes and evaluates resident’s responses to care plan and plans, implements, and adjusts care in accordance with customer focused expectations.
  • Functions in accordance with the standards of nursing practice from the College of Nurses of Ontario.
  • Functions in accordance with the Carefor policies and procedures and the strategic direction of Carefor Pembroke-Renfrew County
  • Functions and reports in compliance with the Occupational Health and Safety legislation, regulations, and Carefor policies and procedures
  • Performs other duties/tasks as assigned by manager
